Job Description - ACTIVITY COORDINATOR 

We are looking for an enthusiastic, outgoing person to serve as an Activity Coordinator at Haciendas at Grace Village. You will coordinate, schedule / plan, prepare and conduct activities for residents on a daily basis, respond to social needs of residents with appropriate intervention.

Examples of work performed:

●      Coordinates resident activities on a daily basis with all departments.

●      Prepares / posts activity calendar monthly

●      Prepare monthly bulletin boards

●      Assists in preparation of monthly facility newsletter

●      Identifies social needs and intervenes appropriately to resolve / meet those needs

●      Establish a strong volunteer program

●      Communicate with the community to provide resources for activities

●      Practices good infection control

Knowledge, Skills and Abilities

●      Ability to plan, direct, and follow through on projects

●      Knowledge of social and activity needs of the geriatric resident

●      Good writing and communication skills

●      Ability to work well with others

●      Ability to interact with various groups within the community

●      Knowledgeable of fire and safety codes for nursing facilities

●      Knowledgeable of Resident's Bill of Rights

●      Ability to Multi-task

●      Computer skills

Position: Assistant Nurse Manager - LPN position for Assistant Living Campus specializing in Dementia

 Haciendas at Grace Village, an Assisted Living Community, specializing in Dementia & Alzheimer’s facility, is looking for an LPN Nurse Manager to work to coordinate and lead an Alzheimer’s and Dementia units with responsibility for planning and coordinating daily resident care.

Job Description:

  • You will provide nursing assessments and direct care oversight, identify clinical trends, implement corrective action and monitor the accurate delivery of medications to our residents, in addition to working with the residents’ physicians and medical personnel on the best plan of action for their continued care. Manage the day-to-day healthcare operations of the community to ensure residents’ healthcare needs are met.

  • Must be knowledgeable of NM State regulations that govern ALF’s, ensuring all state regulations are followed and implemented.

  • Staff hiring and training

  • Perform ongoing assessments/observations of residents’ physical and psycho-social needs.

  • Provide continuity of care for those residents receiving home health care, hospice services, and other third party healthcare-related services

  • Serve as a point of contact for the resident’s family members, vendors and professional contacts in the community, dealing with issues such as scheduling, resident care, service plans, assessments.

Qualifications:    

  • NM LPN license required

  • Experience in geriatric nursing and dementia care (preferred).

  • 2+ years of experience in a health care supervisory role (preferred).

  • Solid computer skills with proficiency in Microsoft Word and Excel.

  • Excellent written communication skills including proper grammar and professional writing.

  • Excellent verbal communication skills with the ability to interact with a diverse group of associates, residents and family members.

  • Ability to apply reason, logic, and advanced problem solving skills to resolve complex and/or sensitive issues.

  • Ability to function as a team leader as well as work as a contributor in a team environment.

  • Ability to organize and manage multiple priorities.

  • Positive attitude, enthusiasm and energy.

  • Strong customer orientation to older adults.

  • Ability to effectively communicate

  • This position requires excellent verbal and written communication skills, the ability to present to, lead and interact with groups using strong public speaking skills. The position also requires office skills including computer skills, and a current working knowledge of different aspects of the campus. Proficient with Microsoft office or other technical equipment.

  • Must have open availability to work of weekends and holidays

  • Must be articulate, highly organized and proactive, with documented success working under stringent deadlines.

Now Hiring: Culinary Aide

Job description

The Culinary Aide is responsible for providing food service to residents and guests under sanitary conditions and for providing assistance in all culinary functions as directed and in accordance with established policies and procedures.

Essential Duties and Responsibilities

  • Deliver food to residents in a timely manner

  • Prepare foods where special dietary requirements are needed; puree, mechanical soft, etc.

  • Assist in the preparation of meals and special events as needed

  • Clean dishes and maintains a clean kitchen work space

  • Prepare a sanitary bucket daily and change out as needed (at least every 4 hours).

  • Log all food temperatures; lunch and dinner

  • Log both fridge and freezer temperatures 3 times a day

Education/Qualifications

  • High School diploma or equivalent

  • Previous experience in food preparation preferred but not required

  • Experience in health care setting preferred

  • Must be compassionate and have a passion for serving seniors

  • Ability to present food in an appealing fashion

  • Strong communication skills

  • Understanding of sanitation and infection control practices preferred

  • Must be ServSafe certified

  • Strong customer service orientation

  • Ability to work collaboratively with all coworkers

  • Must submit to all applicable pre-employment and annual tests per state and federal regulations

Work Environment
The work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee will work long hours and be expected to manage their time wisely. With that being said, employees must adapt to a fast paced environment and proficiently communicate where needed. All employees are required by law to take an hour lunch break when scheduled for more than 5 hours, 5 hours or less employees will take a 30 minute break. Employees are able to take two 15 minute breaks while clocked in as long as you take your required break accordingly, per company policy.

Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is expected to agilely work on their feet and to independently complete all tasks. This position is very active and requires standing, walking, bending, stooping, and crouching all day. The employee must frequently lift and move items 35 pounds or more.
